Product Example Requests

1. Personalized information such as your name, logo, etc. are not included on any product examples.
2. Each item selected comes in limited colors and materials. However, additional color examples may be requested.
3. Product examples do not count as a design proof. If you would like to purchase duplicates of the sample you receive, you must provide the specific design information in your order.
4. The examples you receive may differ from the products shown on this order form.
5. Each request will give you access to our monthly newsletter and email subscriptions.
